Evaluation of Cardiovascular Health Program

Description: This funding will allow West Virginia University Health Research Center to work in collaboration with the Cardiovascular Health Program (CVH) in the Division of Health Promotion and Chronic Disease within the West Virginia Bureau for Public Health to continue the evaluation of the implementation and impact of programs conducted by the Office of Health Services Research (OHSR) to support the use of electronic medical records (EMR) and disease registries in community health centers serving individuals with cardiovascular problems, such as high blood pressure and high cholesterol. The goals of this project are to; (1) Collaborate with the CVH program to evaluate the implementation and impact of the pilot quality improvement program. The program, delivered by OHSR, will provide education and support chronic disease registries and electronic medical records in community health centers providing care for individuals with cardiovascular health problems and including incorporation of the “Million Hearts” program; (2)Develop and implement a process to integrate clinic-level Assessment of Chronic Illness Care (ACIC) scores, Aspirin use, Blood pressure, Cholesterol, and Smoking Cessation (ABCS) measures, clinic policy changes, and clinical indicators to guide OHSR’s intervention for each clinic; and (3)Conduct a survey of clinics working with OHSR and CVH.
