Evaluation of the WV CONNECT/SHAP Program Yr 3

Description: This funding will provide the West Virginia University Health Research Center working in collaboration with the West Virginia Bureau for Public Health and WV Connect stakeholders, to coordinate the program’s evaluation activities and dissemination of project findings (through executive summaries and learning events) so that others in the state can benefit from the knowledge gained from this project. The U.S. Health Resources Service Administration has provided some guidance for the evaluation plan; this along with frameworks for program evaluation put forth from the Centers for Disease Control and Prevention will form the basis of this project. The goals of this project are; (1) Ensure that the SHAP/WV Connect stakeholders have information that clearly describes program activities and logically connects the activities to key outcomes; (2) Capture lessons learned, including barriers and facilitators to program implementation at the clinic-level, in addition to barriers and facilitators to enrollment and health management at the individual-level.
