Program 1815

The WVPRC partners with the West Virginia Division of Health Promotion and Chronic Diseases (WVHPCD) as evaluators for a five-year, CDC funded program 1815 - Improving the Health of Americans Through Prevention and Management of Diabetes, Heart Disease and Stroke.

Program partners across the state build local capacity and support communities and health systems in preventing diabetes and improving cardiovascular health by increasing clinical community linkage, implementing electronic health records, developing a referral system that facilitates access to prevention programs, as well as training multi-disciplinary teams in best practices and how to utilize new resources available to providers.

As the monitoring and evaluation team for this project, the WVPRC utilizes Redcap to track and monitor program progress on a monthly basis while using collected data to prepare regular reports and develop evaluation plans for the WVHPCD and the CDC.
