Community Transformation Grant Year 3

Description: This funding allowed the West Virginia University Health Research Center (HRC) to work in collaboration with the West Virginia Bureau for Public Health, the Community Transformation Grant (CTG) state management team, and other key partners to direct and coordinate CTG Evaluation Activities. The goals of this project are to; 1) revise the CTG evaluation plan as needed, 2) conduct baseline and follow-up evaluation activities, 3) implement and revise performance monitoring tools, and 4) disseminate evaluation data to state management team, stakeholders, and/or research publication venues.
