Evaluation of Diabetes Prevention and Control Program

Description: This funding will allow the Health Research Center, working in collaboration with the Diabetes Prevention and Control Program (DPCP) within the West Virginia Bureau for Public Health, to continue the evaluation of the implementation and impact of programs conducted by the Office of Health Services Research (OHSR). OHSR is contracted by the DPCP to support the use of electronic medical records (EMR) and disease registries in community health centers serving individuals with diabetes. The goals of this project are to; (1) collaborate with the DPCP to continue the evaluation of the implementation and impact of OHSR’s programs supporting chronic disease registries and electronic medical records in community health centers providing care for individuals with diabetes; and (2) develop and implement a process to integrate clinic-level Assessment of Chronic Illness Care (ACIC) scores and clinical indicators to guide OHSR’s intervention for each clinic.
